The V leads from this patient are shown here. The persistent S wave with a small R wave in all V leads is also characteristic of the ECG in patients with COPD. The very small S wave in V1, in association with the small R waves and deep S wave in leads V5 and V6 and with the right axis deviation shown in the limb leads is suggestive of right ventricular hypertrophy.
