The ECG shown here is from a 24 year old medical student admitted with chest pain and shortness of breath. His cardiac enzymes were elevated, but the coronary angiogram revealed no evidence of coronary artery disease. The echocardiogram revealed diffuse left ventricular hypokinesia, consistent with myocarditis. The ECG shows ST elevation in most leads and could be mistaken for pericarditis, acute ischemia or early repolarization. The ECG recorded two days later is shown on the next page.
