The ECG shown here is from a 29 year old patient with ventricular pre-excitation via a posterior septal bypass tract. The delta wave is postive in leads 1, aVL, and V3-6. But, as indicated by the arrows, the delta wave is negative in leads III and aVF and positive in lead V1, simulating the ECG of an inferior wall infarction with posterior wall involvement.
