The initial QRS component (small Q in lead I and larger S waves in II and III) is directed inferiorly and slightly to the right with an frontal plane axis of +100 degrees. The middle component (R wave in lead 1 and larger S waves in leads II and III with the S in III larger than that in II) is directed to the left and superiorly with an axis of -50 degrees). The terminal component (S wave in leads I and II and small R wave followed by a small S wave in III [also referred to as an rs wave]) is directed to the right, perpendicular to the axis of lead III. Its axis is + 210 degrees. The loop encompassing these vectors is inscribed in a counterclockwise direction.
