The most common manifestation of bifascicular block is the combination of right bundle branch block and left anterior fascicular block. The ECG shown here, from a 91 year old male, demonstrates this combination. Try to plot the initial, middle and terminal QRS vectors in the frontal plan and then enclose them with a vector loop.
