This ECG is from a 19 year old female whose chief complaint was fatigue. A grade 5/6 diamond-shaped systolic murmur similar to that reproduced here was best heard in the 2nd left intercostal space where a thrill was also palpated. As in the prior example, interpret the ECG, determine which of the following is the most likely diagnosis and predict the severity of the lesion:
