An example of the ECG changes associated with an acute anteroseptal infarction occurring in a patient with preexisting RBBB is shown in this tracing. The acute infarction is reflected by the ST segment elevation and T wave inversion in leads I, aVL and V2-V5 and by the Q waves in leads V2, V3 and V4.
